Dr. Isaac Condit Hazlett, Burial #3116 in Lot #206 of the Beallsville Cemetery.  Dr. Hazlett was born on Feb. 20, 1840, near Van Buren in Morris Twp. (subsequently S. Franklin Twp.), Washington County, Pa., a son of Samuel and Jemina Condit Forsythe Hazlett.  He died on June 29, 1914, in California, Washington County, Pa.  He married Nancy Eleanor Jennings on March 4, 1867.  He received his medical training from Dr. E.F. Dodd of Washington, Pa., and from the University of Pennsylvania.  His medical practices were located in Scenery Hill, New Salem, Belle Vernon, Uniontown, and Monessen, Pa.  He was a member of the Fayette County Medical Society from the time of its reorganization and served a year as its President.  He was an Elder in the Presbyterian Church.  He shared ownership of the cemetery lot where he is buried with his brother-in-law, Rev. M.A. Rigg.
